The trays control where the gel goes. A tray moulded to your teeth from an impression sits against every surface with an even layer of gel held in contact. Generic strips and boil-and-bite trays do not follow the contours of individual teeth, which means the gel contacts the flat front surfaces and misses the curves near the edges and between teeth. That is the source of the patchy result patients often report from over the counter products, where the middle of each tooth lightens and the margins stay dark.
The gel does the actual work. Prescription strength whitening gel contains a considerably higher concentration of active ingredient than retail products, which is why it produces a greater shade change and reaches it faster. The higher concentration is also why it is dispensed through a dental office, since it should be used in a properly fitted tray under a dentist's guidance rather than applied freehand.

Whitening is not permanent, because the causes of staining continue after treatment ends. How long results hold depends almost entirely on habits, and the range runs from several months to a couple of years.
The first 48 hours after whitening deserve particular care, since enamel is more porous immediately afterward and takes up pigment more readily. Sticking to lighter coloured foods and drinks for those two days protects the result you just achieved.
Beyond that, routine cleanings do a good deal to maintain brightness by removing surface deposits before they build. And because you keep your trays, refreshing the shade later is a short course with touch up gel rather than a new treatment. Most patients find a few days of top up once or twice a year keeps things where they want them.
